Les opiacés d'origine synthétique, c'est-à-dire n'étant pas synthétisés à partir de l'opium, sont désignés sous le terme opioïdes. Le cerveau humain utilise certains opiacés naturels (les endorphines) comme neurotransmetteurs. La plupart des opiacés entrainent une très forte dépendance physique, à l'exception du lopéramide qui ne franchit pas la barrière hémato-encéphalique.

These agents work by binding to opioid receptors, which are found principally in the central nervous system and the gastrointestinal tract. The receptors in these two organ systems mediate both the beneficial effects, and the undesirable side effects.
